I just wanted to comment on Jan's discussion of the scope of practice, and
perhaps take it a little further.  My practice is in a triage service,
specifically a telephone triage service.  Breastfeeding is part of what I do as
an RN in this job, and having my IBCLC has turned out to be everything I hoped
for.  The other nurses seemed to finally believe what I had been saying all
along!  Our general information has improved considerably.  The reason I am
commenting here is that my practice has to do with the moment--getting the
Mom's through the crisis and breastfeeding throught he night, and getting them
connected to someone who can help them past that stage.  I used to be really
good at the rest of it but it has years since I worked in a post partum floor
or did general breastfeeding counseling.  Now I try to be sure each caller has
the phone number of someone to reach in the AM, or as soon as reasonably
possible.  At this time of the year, when we are reviewing things in general
and when the recert info comes out, it just seems right that we should review
our breastfeeding practice too.  Naturally, some specialization is going to
come into play due to our specific jobs, all with the context of the IBCLC.
