The Breastfeeding Answer Book defines diarrhea in the breastfed baby as 12-16
stools per day, watery stools with no substance or stools with an offensive
odor. Green, watery stools in the absence of fever or other signs of illness
they associate with lactose overload (previously know as "foremilk/hindmilk
imbalance" :-) ). I use 12+ most commonly as my clinical definition of
diarrhea.
