If all the conventional things have been tried to relieve engorgement (ice, ibuprofen, more frequent emptying, cabbage-see the textbooks) then please run to your nearest breast surgeon for a full evaluation including mamogram if you have not had one.  You do not mention how old the baby is, but I recently had a client who had weaned to one sided nursing for this kind of repeated inflamation, only to find the problem did not resolve and it was an intraductal cancer.  Another patient I know of went to a breast surgeion who prescribed nitroglycerine paste topically over the area and it cleared up after that.  
This is definitely a red flag if beyond the early weeks.
