 I am not sure there is a "clear cut" answer.

 Strict dairy elimination includes ALL dairy in any form - and I think that
when there is serious concern the  first step is total elimination.

  When one is going to re- introduce one can carefully try the different
avenues and see what happens. Sometimes foods in one form (ie: a glass of
milk) is an irritant and in another form ( baked) may not be.

    The tremendous amount of variables is what makes the whole subject so
difficult and time consuming.

     I think that you might find food for thought by maureen minchin a
helpful book for these queries.
