The 4/1000 was quoted in Cunningham and Jellife (whom I briefly worked with
at UCLA).  I believe it originates from rogan's study "Cancer from PCBs in
breastmilk? A risk benefit Analysis."  Pediatr Res 25:105A, 1989.

The 1/1000 SIDS reduction is from Damus, K, Pakter, J Krongard E et al.
"Postnatal medical and epidemiologic risk factors for the sudden infant
death syndrome."  in Harper RM and Hoffman (eds).  SUDDEN INFANT DEATH
SYNDROME.  NY: PMA Pub, 1988. pp 187-201.
