This is the very sad story of a 5 month old baby who received a
breakfast containing milk at his nursery (day care) and died as a
result.

Other press reports made it clear the baby was breastfed.

His mother had told the nursery of the allergy (made apparent when he
was given formula in addition to breast some weeks before and had to
be hospitalised) . But the staff ignored this/forgot about it and as
a result this little boy had an anaphylactic shock and died.
