I seem to remember a topic on Lactnet sometime ago addressing the idea of
having mother's sign consent forms if they want their babies to be given
formula in the hospital.  We are looking at this as a way of decreasing the
amount of supplemental bottles that are given.  Does anyone have an example
that you could send to me?
