Our hospital is using a document that mom acknowledges she has been informed about benefits of breastfeeding/hazards of formula.   Yes nurses can document, and no, informed consent forms don't always hold up in court, but it is a way of helping to ensure that education has been done before the first feeding of formula.  Mothers sign a lot of forms indicating that they have been educated and informed about such things as epidurals, circumcisions, hep-b injections, hearing screens, etc, back to sleep, car seat use, etc. This is no different.

Our hospital was unable to find anything written in the Joint Commission's guidelines that specifically states that the mother must sign the document.  The only requirement was that the practitioner document the date/time of the discussion w/ the mother in the mother's chart.  Our Pediatric team then makes note in the newborn's chart that they viewed documentation of this discussion in the mother's chart.

Anyone else find otherwise?

Why must this be so complicated?
