There is research that has been done on this question. Look in the
archives
of Journal of Human Lactation, and look in a medical research database.
Then you can contact the authors to get copies of their questionnaires.
Usually people are willing to share this info with other researchers,
because
if their questions are good questions that prove reliable, then using them
on other studies can help support their findings. Besides it's
professional
courtesy. Often the basic questions are reported in the articles written
about the research, but it's good to get the survey instrument itself if
you can.
Another place to look is govt research agencies like CDC and others that
do surveys. Their questionnaires are in the publice domain, probably on
the
web, and are usually well tested.  It will take some poking around on the
web and the library to get the questionnaires but they are out there.

For purposes of reliability and validity you are better off using
questions
that have already  been used and adding a few of your own.
