Dear Heather and All,
 Just to respond to Heather's post, in my experience, unfortunately I have never been wrong about observing someone who I thought had insufficient tissue and the breasts that Heather described and would not continue to be able to fully supply their baby.  The point at which it all crumbled was often at a different place- some around two weeks, some stringing it out until about 6 weeks.  Those that managed to continue to around the 6 wk point were so exhausted at that point because to keep their baby gaining the bare minimum they were usually pumping and nursing times 5 more than normal and still dealing with a hungry kvetchy baby who never settled in addition.
It is so sad, because there is really so little we can do to remedy the situation.
