I personally had a very hard time with lots of different nipple problems,
but vasospasms was one of the most painful.  In fact, I get uncomfortable
just thinking about it. The single thing that helped me the most was to
protect my nipples from all tactile stimulation, except for when I was
nursing.   Each bout with the problem would start with them clamping down
and blanching white (ouch), upon coming out of the baby's mouth but, after
that happened a time or two, it got where the slightest bit of stimulation
would create the spasms.  I found that it helped to keep several layers of a
soft, fluffy, prefold cloth diaper covering them, held there loosely with
the flap of my nursing bra lifted up but not fastened, whenever the baby was
not nursing.  I tried to keep everything but the inside of the baby's mouth
from ever touching them at all, and also would take the diaper I had used as
a cover and put it inside my clothing, to keep it warm, then put it over
them the second the baby unlatched.   Every time they clamped down set
things back but, if I could really avoid it for a few days, it would stop,
until the next bout.

I have heard a few LC's say that nipple vasospasms don't really exist, but
have just become a catch-all for cases of unexplained sore nipples, but I
can testify that it is something very specific, very real, and very painful,
and the last thing mothers dealing with them need to hear is that they don't
really exist, or that they just have sore nipples.  Besides the pain, having
the nipple tissue contracted so hard that blood supply is cut off closes the
ducts, too.  Anyone dealing with that problem has my sympathy!
