I am sorry, I don't remember earlier posts.  Have you considered that this
babe is trying to compensate for a very fast flow (OMER)?  Would what we
call 'posture feeding' here be worth a try?  This is where the mother feeds
the baby while reclined or even lying flat on her back with the baby lying
tummy to tummy, along the mother's body.  It is important to end the feed
sitting up so as to avoid blocked ducts. 
There is a podcast here http://www.lrc.asn.au/podcast/podcast.html that
discusses this.  It is designed for mother-to-mother support so not
technical but the information is good.
