I also don't like to restrict baby's hands when nursing. Usually I do what
most of us do, place the lower arm in a position that it is under the
breast and hugging it. Mom's usually can manage the upper arm while getting
babe latched and then I encourage her to let the baby's upper arm also come
up and "hug" the breast. In the rare case where mom and babe just can't
manage the upper arm to obtain a latch, I show her how to slide the hand
under the strap of the nursing bra for a temporary "handcuff". And I do
mean temporary, cause all these little guys can get that hand out in about
45 seconds- but it does buy mom time to latch the babe.
  On the swaddling arms to the side issue..too many years as a NICU nurse
has really caused a general aversion to this technique for the most part.
Even in the most disorganized infant, neonatal therepists who might swaddle
generally do so with arms up and only swaddle for a brief time to initiate
a feeding. As soon as the baby "organizes" and can rhythmicly suckle well,
all therepists I've seen immediately begin to slowly free the infants arms
to promote normalization . Even when the infants are in their cribs or
isolettes, we always provided time with the baby being in a sidelying
position and a body roll to cuddle around to prevent abnormal constriction
of shoulder muscles...
 IMHO, if an infant or child needs to be swaddled with arms to the sides
because they cannot routinely organize and normalize without restriction, I
really hope someone in developemental peds is also evaluating. Just a
thought.
