Hi everyone. A local hospital is working with community agencies to create  a 
lactation program for at risk populations.  They are working on a grant  and 
have a few things they still need. I suggested I come to those who may have  
already been there done that and ask for some pieces we think would make the  
work go more smoothly.  What we are looking for is:
1.  A Logic Model that is geared to breastfeeding support 
2.  A questionaire/tool that is short and can be used to assess  outcome.  
Thinking of questions that look for information about attitude,  success, 
feeling supported, etc.
 
The goal is to provide prenatal and postpartum through 12 weeks support for  
those who are willing to participate.  This will be a great program if it  is 
funded, and much needed in the community.  If you have worked on a   grant for 
lactation support in the past and needed to use a Logic Model and/or  have 
tools for collecting data you think would be helpful, can you please email  them 
to me off the list.
