Are you looking to evaluate attitude towards breastfeeding? Or, are you more interested in how confident the doctor feels about providing breastfeeding information or support?  Or is it more just general knowledge?  

In the first instance, if you ask a doc if they (made up question) "support breastfeeding as the best form of infant nutrition?" Agree/Disagree you are going to get a lot of biased answers to Agree. Very few people, unless they work for a formula company, would say that breastfeeding isn't.  There is (thankfully)a growing social desirability bias in asking about attitude.
In the second instance, the breastfeeding efficacy scale - while normed for moms - could possibly be reframed as a useful scale for docs.  
If you are interested in general knowledge then I would suggest taking some of the post tests from the AAP curriculum on breastfeeding as a quick knowledge assessment.
