My workmate, Robyn [IBCLC], has been asked by a mother to tell her whether 
it is worth continuing to express for her 10 wk baby, She is an inpatient, not in 
maternity, but is keen for some professional comment. Read the story from 
Robyn and some feedback would be really appreciated.

'Yesterday i reviewed a 41yo mum with lymphoma who has a 10week old baby. 
He is A/F at present and receiving care from his grandma while mum is 
receiving chemo. She is currently expressing TDS with a yield of 15ml EBM at 
each pump. That is, a total yield of 45ml/24hrs. The next stage of treatment 
entails an autologous transplantation of stem cells which will include further 
chemotherapy, total body irradiation and blood transfusions over a period of 1 
month or so.  Her question was should she maintain her hope of breastfeeding 
again by continuing to maintain a supply by pumping or accept that future 
breastfeeding is out of the question.  IE Refocus on other issues of her life.  
Due to the fact i have not encountered the above issues in my previous 
practise, i asked her to allow me a day or so to do some research and to 
consult with other IBCLCs, before returning to her.  Have you encountered any 
similar cases and what would you consider to be the optimal course of action 
in this situation?'
