Dear Folks:
Thank you to everyone who responded to my post, both publically and
privately.
I am wondering if I didn't do enough assessment at first. I know this
mother, taught her in a nursing mother's training class. She is outspoken,
very educated, thinks a lot. When she brought her baby to me, for CST more
than for BF help, I took her word for most things because she had nursed
before, because she is a counselor, and because she is a strong outspoken
intelligent woman.
I don't think I asked enough questions. Her baby, at 10 weeks, weighed 12
pounds 9 ounces, a 3 pound gain in 8 weeks. This isn't such a huge gain that
would point to oversupply, now that I think about it. However, baby's
behaviour at breast (which I observed) plus the green stools plus the
enormous and constant spitting up led me to recommend things to deal with
oversupply.
She also refused to consider somethings. When I recommended her seeing a
pediatric allergist for the baby to be skin tested for allergies, she flatly
refused. I pointed out that his response to the elimination of soy, plus her
family history would be a good reason to suspect further allergy with his
fussiness. I suggested that prick testing would enable her to be precise
about what foods, if any, should be eliminated. She wouldn't go for it.
But now she has him at a pediatric allergists', because she thinks there
is something wrong with him at 2 hours after feeds.
I will talk with her again.
Again, thanks for a forum where mistakes can be used for the learning of
all.
    Warmly,
Nikki Lee RN, MSN, Mother of 2, IBCLC, CIMI
