Nancy, I am looking for exactly the same thing.  I have a private practice
and in my history form I ask more and more of these questions (thanks
especially to the latest work of Lisa Marasco and others, but going back to
all my references).
If no one has published this, I believe it would make a wonderful JHL
article.
