The first thing that comes to mind in friction.  Has tongue tie been
ruled out?  Some such babies can get the tongue out over the
gumline, but tension keeps pulling it back in, then baby extends it
only to have it drawn back in again, etc. etc. etc.  So a quick feel
of baby's suck may not be enough.  You may need to leave your finger
in the mouth a little while and see if the tongue stays over the
gumline.  (This would, bo of course if you have been unable to make
any determination by visual assessment.  Go from least invasive
toward more invasive only to the extent needed to verify or disprove
it.)
If all else fails, and there is no tongue tie to correct, this is
one case where a nipple shield may help by providing a "cushion"
between baby's tongue and mom's breast.
