sure it is even my place to suggest this is possibly the issue, because a) I
am not a diagnostician and b) not an IBCLC ( yet) and c) if I hit her with
that, then what? no one to follow up with? would her midwife be an
appropriate voice for this ?

**I think it is already a good thing that you noticed all this, after all
the disappointments she experienced with her other three children. Your
knowledge may help this mother to understand why she didn't succeed the
previous times. On the one hand, she may be 'hit', but on the other hand
maybe more by the fact that noone told her before, than by you telling her
now. Knowing what is wrong, may make it easier for her to accept the fact
that she wasn't able to breastfeed. It might even support her
selfconfidence, knowing she did right before, but simply couldn't help the
failing lactation.
I don't know what your relationship with her is like...? Are you close? Does
she trust you (enough)? If you carefully express what you saw, just stating
facts, would it be an idea for her to get in touch with the lc again, who
could comment on your findings...?
I find this interesting. We deal with many women and there are those app. 2%
out of 100 who have primary insufficiency; unfortunately, we can't always
make it work... ;o)
