I have 2 friends (who are also my patients) who are exclusively pumping for
their babies now.  The one, the mom of triplets who lost 2 babies early on,
is doing well with her supply, and better with her sore nipples.
The other had a sort of series of unfortunate events that has drastically
dropped her supply.  Her little boy had G-tube surgery and then was
discharged from the hospital.  The 2 days prior to discharge she missed some
pump sessions as she was staying in a family room caring for the baby
totally in preparation for discharge.  I arranged for her to get a Lactina
grant pump from WIC, which her husband was supposed to pick up the week
prior to discharge, but he didn't get it, and she ended up having nothing
but a Medela single hand pump for 2 days.  Now, her supply has dropped from
35 or so oz a day to 10-12 oz.  I have her pumping as frequently as she can
for a few days (averaging about every 2-3 hours during the day, with one 4
hour break at night), had her start fenugreek, and yesterday wrote a
prescription for Reglan.  She was heading to stay with her mom for 10 days
or so after seeing me yesterday.  She has used her freezer supply and had to
give formula several times yesterday, which the baby doesn't tolerate well
at all.  Her mom is still nursing her 13 month old baby sister (She is the
oldest of 11!) so she is hoping to have some donor milk from her while at
home, and she will have good support both with caring for her baby and 2
year old and emotional support as well.  Any other ideas to help her get her
supply back?  Baby is 7 weeks old and unable to breastfeed at this point.

Also, this mom is interested in not using commercial formula if she has to
supplement.  She has a brother that had goat's milk as a baby and wondered
about using that.  If I remember, there has been discussion here about
goat's milk being too high in solute load for babies.  Any good links to
info I could send her?
