We're back to relative risk again.

What is needed is a comparison of the risks of feeding infant formula to a healthy at-term infant vs the risk of feeding donor human milk, with the latter broken down further into categories such as 1) unpasteurized milk from a screened or healthy donor/wet-nurse, 2) pasteurized - and possibly even a breakdown based on different methods of pasteurization and screening prior to pasteurization.

This information is necessary and important - we now have two countries who have issued warnings against "informal" milk sharing - Canada and the United States (Health Canada and the FDA) without any specific references to the evidence base used to issue the warnings, or any attempt to provide information for parents or healthcare professionals on relative risk. There are no guidelines that instruct on how to reduce risk.

We also have had a number of health care professionals in Canada and the US who have been quoted in news coverage as saying that for healthy infants, infant formula presents less risk than donor milk that is not from a milk bank. And in France we have had a health care professional state erroneously that informal milk sharing is illegal.

These issues are not going to go away.

I know there is good research for the preterm infant but my physician partners are not in agreement that the benefits are the same for the term healthy infant. I am aware of the exclusive breastfeeding benefits and the statement in the Breastfeeding Task Forces recommendation that we use human milk, then donor milk and formula as the last choice. Can anyone assist me?
