I am dealing with a situation here which I have not seen previously and
would like to hear from anyone who has dealt with a similar problem.
A young woman was admitted with PV bleeding and uterine tightenings 4
days ago. The tightenings are continuing but the bleeding has ceased.
Yesterday she developed firm/full breasts and started leaking colostrum.
There is no sign of an infection and both breasts are affected.
Examination of her breasts would indicate Lactogenesis II has occurred
at 34 weeks - antenatally.
Apparently her pregnancy is not showing any abnormal readings ie: by
scan and EEG - but I have to feel concerned about the progesterone
levels given the breasts reaction. The doctors do not seem concerned
despite the fact this woman has had an intrauterine death previously at
23 weeks.
Should I press the issue further - what is the likely course of this
reaction for this woman? Will this now resolve and when the baby is born
(hopefully a few weeks away) - will her breasts respond again (I suspect
so - if things proceed) I have never seen this scenario before but I'm
sure someone out there has!
