Don't you mean that the general rule is that when mother is alert enough
after the general anesthesia, it means that the anesthetic dose has been
eliminated from the mother's blood stream (or blood compartment as Tom
Hale would say), thus can not get into milk. It may be stored elsewhere
in the body until it is gone in about 11 hours. At least that is what I
have understood about this for many years.

Subject: Ketamine

Ketamine has a half-life of around two hours, which means most would be
gone in about 11 hours, not 12-18 hours. Of course, 11 hours is too long
to wait to begin breastfeeding.  I would try to "cheat" as much on the
underside of 11 hours as possible to begin breastfeeding, or even
better, follow the general rule that when the mother is alert enough
after the general anesthesia, it means that anesthetic doses have been
eliminated from the mother's body, and she should then be able to
