Hi All,
Wondered what you all thought about extended use of nipple shields (like
longer than a few weeks) and what you thought about how a mothers milk
supply might react.

For example - mother of twins, BF exclusively now with nipple shields for
both babies. She is able to pump maybe one - two times per day, is taking
Blessed Thistle and Marshmallow/Fenugreek comb, and the babies are at the
breast all day long (with breaks in between for sleeping).  Each baby is
gaining approx 2 oz per day and with pre-post weighs each is taking in 2-3
oz per feed.  Babies BF at least 10 -12 times per day, have plenty of wets
and poopies and are really starting to fill out.  She also has psoriasis on
the nipples and has suffered terribly with pain with each of her other 3
babies.  However, she has found that with the shields, she is having NO
problems with the psoriasis.  She is wondering how long she could use the
shields for BF (without compromising her milk supply) to help with NOT
having psoriasis problems with the twins. Oh, and she has an abundant milk
supply at this time.  After the babies are done BF and when she has time,
she is able to pump at least 1.5 + oz of milk from each breast (and this is
after the babies have been at breast BF for at least 30+ min.).  These
babies are not supplemented at all.  The babies are not yet able to BF
without the shields, but mom continues to offer the breast sns shield
frequently each day.

Would you worry about this moms milk supply using the nipple shield long term?

One more question:

If the mother(any mother using a nipple shield) is not protecting her milk
supply in some way (ex. herbs, pumping after feeds, etc) how long would it
be (in general) before she would see a drop in the milk supply?

OK, I lied, one more question :) :

If the milk supply is based on supply - demand and a baby is able to gets
what s/he needs at breast every feed while using the shield, then why would
the milk supply drop because of the shield?  If the demand is there then the
supply should also be there even with a shield right?

Lots of questions - hoping to be educated with lots of answers!  :)
