Please be aware that we are talking about 2 different uses here.  In
any kind of "suck training" (or whatever you want to call it), it's
appropriate to have the finger drawn to the same point the breast
will reach and that is how I do it also.  Using the knuckle is to
give baby something to do while waiting for mom or for those
instances where baby seems to need just a few more comfort sucks
before falling asleep.  I also suggest it when I know that a parent
will use a pacifier in spite of the reasons I give for avoiding it.
The advantage is that the baby is held (or at least the person is
next to the baby) while sucking on the knuckle as opposed to simply
having a plug popped in the mouth and then being left with no form
of human comfort!
