When I see "very red" nipple/areolas, or a definite rash (ie the redness has
a definite edge to  it) or the mother says the rash is "spreading" or
"itchy" -  I would be more suspicious of a dermatitis than nipple thrush.
The dermatitis may be an "irritant dermatitis" rather than a true allergy.
Stopping the cream for a couple of days will usually give you the answer.
