Apologies in advance if this has already been stated.  We are having
trouble getting our incoming mail, but can respond to what's already
"delivered" so I am behind several digests I am sure.
For "normal" nursing, we generally recommend mom support the breast with
fingers under - often referred to as "C" hold.  However, when baby is
overwhelmed with the flow, this is one time she may find it helpful to
use the "V" (or scissors or cigarette) hold.  By compressing the breast
between her index and middle fingers at the start or when a letdown
occurs, she can reduce the immediate flow to the baby.  Of course, she
needs to still be sure her fingers are far enough back on the breast to
allow baby sufficient room for an effective latch.  If she needs to
repeat for multiple letdowns, she can concentrate on lifting the breast
from underneath with the middle finger while keeping the index finger
"at the ready" for another scissors compression as needed.  Apparantly
what this does is to limit the flow from those ducts that are being
pressed by the fingers.
