My understanding is that we see the real difference in fat content only if
there's been a longer interval between feeds, and that the mother who nurses
several times an hour has no significant difference because the milk never
has time to separate.  

If I were a mother being encouraged to make sure the baby "gets the
hindmilk," I'd assume feedings need to be long, and because they're supposed
to be long I'd be inclined to space them out, further compounding my low
supply.  I'd be better off with the image of fat swirling near my nipple by
the end of a meal and then slowly retreating into the depths of my breast,
and therefore wanting to "keep my milk rich" by nursing really often.
