Hi Rita, I am CLEC and just finishing my LC course at UCSD with Gini Baker, RN, MPH, IBCLC. In my studies I've come across refusal to breastfeed on one side several times. In Cadwell & Turner-Maffei's book, Breastfeeding A-Z (2006) pp. 127-128, 277-278, regarding breast refusal they advise "Prompt care needed for repeated refusal of one breast. A mother whose baby refuses the same breast should be referred for breast evaluation, and should be routinely monitored for breast cancer. Cancer has been diagnosed as late as five years after persistent breast refusal. This phenomenon is referred to as Goldsmith's sign."

The fact that mom has a previous history of cancer is very concerning as pregnancy hormones are known to enhance cancer growth. I think a referral to her Oncologist and or OB-GYN is needed here asap for futher evaluation of the mother's breast for potential metastasis.  Hope this helps. Please let me know what the outcome is as this a very interesting case.
Thank you,
Deb Stich, RN, BSN, PHN, CLEC

PTP: Mom exclusively breastfeeding baby at day 15 day. Baby was born at 3.1 kg. Dint appear to loose weight after birth. Day 3, day 5, day 7, day 10, day 13 and day 15
weighted 3.1 kg again weighted in different weighting machines shows same. Baby appears to be well hydrated with 7 diapers and 5 to 6 breastmilk stools. This no changing weight chart has stumped me. Has anyone else seen this?

Thank you for your insights in advance.

Subject: polymastia

Hello Lactneters:
PTP. I saw a client today with an 11 day old baby, her second. She is exclusively breastfeeding and reports having a lot of milk. She has accessory breast tissue with a patent nipple under one arm. Her ob told her to not rub or squeeze it and to keep it bound up. Mom reports it being very full, hard, warm to the touch and painful. She says that she sometimes does squeeze milk out when it is too painful to stand. Is it possible to develop mastisis in acessory tissue? Would expressing just enough to stay comfortable an
 appropriate suggestion?
She also seems very interested in having it removed. Any information/experience regarding this is welcomed and appreciated!
Thanks for your thoughts and insight.
Best,
Amy

Subject: one-sided breast refusal

Hi all,

PTP.  G2P2 Mother of 9 month old is experiencing biting and pulling away with some breast refusal on the R side that started 3 days ago.  She thinks it started when baby bit her and she startled baby with a "no" to the baby.  Baby will not bite L side and nurses very well on L side.  Baby will not take milk pumped from R side either.  Mom states she was able to sneak baby onto R side and sneak a bottle of EBM from R side when baby was very sleepy.  She has also tried to start baby on the L side in cross-cradle hold then
slid her body to the baby to nurse on R side without moving the baby at all but baby instantly pushed away.  Baby has 2 bottom teeth and no evidence of new eruptions.  No recent illness in baby, is reaching all developmental milestones, eats solids from the table three times per day, nurses 6 times a day, takes 2 naps a day and sleeps 11 hours at night.  Mom had cancerous tumor removed from kidney in December but is healthy
otherwise.  No complaints of peau d'orange, lumps or breast tenderness.

I suggested she continue to try to offer the R side as she has been doing and continue to pump to maintain her supply on that side.  What am I missing?

Thanks for your wisdom and time.

Rita, RN IBCLC

Cabbage!!!!
I would not consider having it removed at this time.  Will
involute.  Hopefully can have it removed between babies.  She needs to see a
good breast surgeon for the procedure, not an OB (remember -waist dwn theory
of practice:-)  Pat in SNJ

Need to weigh consistently on one scale with clothes off.  Going from scale
to scale and possibly in different clothes can tamper with the whole
picture.  Pat in SNJ
