I work in concert with the pediatric staff, whoever is on call, when I see
jaundiced babies or babies who aren't thriving.  About a year ago I did my
first referral for a frenotomy.  Brought it up with the chief attending
pediatrician on Friday, when he pooh-poohed the idea, saying we had stopped
that nonsense years ago.  I got him updated information, and the dyad came
back on Monday determined to have it done, and he had changed his mind in
light of the new info.  It was done in the ENT outpatient clinic less than
30 minutes later with immediate results.  Next time I found a baby with
ankyloglossia was about 6 months later; I called the on-duty pediatrician
and the frenotomy was done within the hour, also by ENT and also with
immediate results.  Two days ago I wanted to refer a third baby for it.  The
chief was once again on call, and had a new resident with him.  They
observed the tongue, were suitably impressed by the extreme soreness in the
mother, and he referred to ENT, this time sending the resident along to
observe.  'Seems like maybe we need to review how to do this ourselves, so
you might as well go see how it's done.'  Both docs came by today to hear
how the dyad was doing, and I was so pleased to be able to report that latch
is better, baby is gaining (at long last) and mother is no longer in pain.
I wish my entire workday had been like this incident, of course.  But it
sure helps when things are going badly in general, to have a positive
experience among all the fiascos.
