I have permission to post. A coworker saw a mom  today that she feels has 
calloused nipples. Mom is a 41yo primip whose baby is  now 4 mo and who had sore 
nipples for the first 2 weeks that healed. She came in  today for a consult 
for blanched painful nipples that occur very irregularly but  just in the 
evening. When she feels the pain, she lowers her bra flaps, sees  that her nipples 
are blanched and then the color returns. My coworker watched  a  feeding and 
feels that the latch looked correct. Upon doing a digital  exam, she noted a 
high arched palate. Mom is aware of pinching with BF but has  felt that from the 
beginning and now it does not bother her.
    Her question is this: is there any treatment for the  callouses on the 
nipples. She has tried different hydrogel pads and breast  shells to reduce 
friction rub but does not care for those products. I also  suggested working with 
this mom on trying to get an asymmetric latch. I was  wondering if the saline 
soaks with some gentle debridement after to soften the  tissue. We would 
appreciate any other suggestions.
