Andrea, I have seen this a couple of times. The most recently with a 
child with Williams syndrome. In another child a few years ago it was 
a rare mitochondrial disorder..  In both cases it took several months 
to reach diagnosis and the lack of weight gain despite adequate 
intake was the earliest symptom.  Both mothers had lots of milk, one 
was an experienced proven producer.  Both mothers were blamed for not 
feeding frequently or adequately and it took some time before they 
were 'heard'. Other symptoms were 'masked' and may have shown faster 
in artificially fed infants. Certainly moving these children to abm 
led to an immediate and in one case dramatic increase in symptoms.
