There can be many variables here.  Yes you do want to make sure that your
scale is calibrated correctly.  Given that - perhaps the baby is not transferring
milk - how many wet diapers, how many stools and what do they look like.
Could the baby have a congenital problem that would cause excess burning of
calories - so that the baby is transferring milk but also burning through calories?
 It might be interesting to see what the test weights are before and after a
bottle of breastmilk (you already said she was giving bottles) and using
techniques or nipples that would cause a slow flow - the baby seemed to be on the
breast for a while so rapid transfer of fluid would not be the best way to
check.
