I am an IBCLC working in the Public Health Nursing Program and currently involved in updating some of the processes in our breastfeeding clinics.  These are staffed by Public Health Nurses and they are asking for a more efficient documentation system than the narrative note charting format they are using.  Does anyone have a documentation form suitable for use in this setting?  I am hoping that one was developed by you and/or your institution that you are willing to share.
