Hi all,
A fellow ABA counsellor has asked for help with a mother she is
counselling and has given me permission to post this case to Lactnet,
for some more opinions and expertise.

Case:
1st baby, 4.5 months, gaining heaps (born 4.5kg now 8.3kg), weight
gains in the region of 400g+ per fortnight, mother with continued
excessive leaking and prob oversupply issues.

Mother having continued bleeding post caesar. Past history of
endometriosis, this controlled by continuous combined pill (no days
off hormones), fell pregnant immediately on ceasing pill.   Bleeding
now is about 3 panty liners per day, some days none, bright red in
colour.  Obstetrician has done ultrasound and tried antibiotics x4
and minipill started at about 11 wks, with nil improvement.  Minipill
started in response to continued bleeding, not for contraception per
se.

Obstetrician has ruled out retained placenta (via ultrasound and
because she did the caesar) and now infection (via swabs and gave
antibiotics), and now suggests combined pill or stopping
breastfeeding, and then D+C if these measures do not work. Dr
thinking that there is a blood vessel prevented from healing by
breastfeeding hormones. Second opinion sought and same advice given.

Mother has spoken to state lactation medication service, who advised
that issues of combined pill less after 12 weeks, and also if good
supply already (ie worse outcome for breastfeeding if already
compromised).  I read Lawrence which seems to concur with this.

Mother wants to keep breastfeeding (>1yr, until he wants to stop).
She wants to know the mechanism of effect the combined pill has on
lactation.  Minipill does not appear to have had any effect on
supply, mother would not mind if supply was slowed a bit.

Can anybody help with
- confirmation that it is possible breastfeeding might be having
effect on blood vessel
- references, websites, anything else that might help direct this mum
re combined pill and breastfeeding.
