In addition to anatomical causes already mentioned, I've seen it when
baby has unfortunately learned to suck on it's tongue in utero.  Such
habits have already become somewhat ingrained and are not always quick
to "unlearn", but can be with some time and effort-whether you call it
"suck training" or something else.

It would also be interesting to explore whether the cause of the
respiratory problems can be the same cause as the clicking (anatomical
variations more likely) or whether it is coincidence that they occured
at the same time.
