I agree that attention to the basics (positioning, latch and management) 
is primary! That said, the pituitary is very sensitive to interruptions 
in blood flow. If mom suffered some pituitary injury during her brain 
surgery, she likely would have had other symptoms of pituitary 
malfunction before her low milk production. If she responds positively 
to increased milk removal, then we can consider her pituitary functional.
