in my experience sometimes in early postpartum, maybe due to IV fluids and
other birth interventions --> edema, no amount of massage and expression will
bring about drops of colostrum, ...I am going to guess at one out of every
six or so very early pp moms I see, pumping is usually even more less likely
to result in milk drops at this early stage for these moms, this usually on
day one or two,

I make a point not to convey anything negative to the mom, quite the
opposite.....

for example, as part of the routine breast and nipple exam if colostrum is
expressed yea! if not I can usually say that I can feel the breast "filling",


and in either case, colostrum expressed or not, I tell moms that a well
latched baby can get colostrum breastmilk out WAY better than any pump and
better than I can with my fingers.

of course there are those cases where there is a really significant delay or
ultimately inadequate production.......
