Hi Cathy,

You said the baby was 34 wks 5 days gestation and "was fed by parenteral nutrition for 16 days...then they began to give him very small amounts of motherīs milk and little by little the baby has tolerated larger amounts, he is three weeks old now, and he is being fed 27 ml on schedule."
As a former nicu nurse and now continuing to work with premature infants for over 30 yrs, I would have to say this was/is a very sick infant. Certainly c-section birth and antibiotics can impact the establishment of normal gut flora. However, there are likely very good reasons why these things were done. It is very unusual for a baby over 34 wks to have such a long period of only parenteral feeds and such a slow progression to oral feeds, now at 3 wks old. I am not sure about the smears for the stooling, but perhaps this baby has a primary gut problem or had sepsis or severe respiratory compromise in the beginning and is just now getting well. At this point the skin to skin holding will be excellent, the mother should be helped to develop and protect an abundant milk supply, and continue to advocate for her baby to be breastfed/breastmilk fed. I think she will need a lot of help and support to establish full breastfeeding also.
