The Thinking Woman's Guide to a Better Birth, by Henci Goer (written
for consumers, but good info for practationers, too)>>

If I could afford it, I’d buy dozens of this book so that I could give one
to every pregnant woman I see.

But I just wanted to mention that before the consumer version came out,
Goer did write one geared toward professionals: “Obstetric Myths versus
Research Realities.” Copyright is 1995, though -- would be nice if the
publisher were interested in updating it.
