As many are acutely aware, breastfeeding by HIV-positive mothers is actively discouraged in North America. It is the norm to see "NO BREASTFEEDING OR BREASTMILK", (typically in all caps, underlined, with exclamation points) in a baby's chart if his/her mom is either suspected or confirmed to be HIV-positive. Rather, infant formula is provided and little to no counselling re: risks, benefits, or lactation cessation is typically provided to these mothers. The WHO has some interesting information re: HIV and breastfeeding and there have been a number of studies published that address the risks of transmission (i.e., mixed feeds, etc.). For additional current information you can see the most recent issue of the Journal of Human Lacation, which has a few articles on such subjects as using heat treatment or chemicals for the milk of HIV-positive mothers.
