I haven't yet, but I am really tempted to start putting a check mark in the 
box if mom chooses bottle or "both"! (a check is abnormal and a circle is 
WNL).   I say this frequently to the nursery nurses and they all sort of 
giggle (uncomfortably).  Have to discuss this this with my boss.  If all of 
us do this we would present a united front to the hospital. :-)  Pat in SNJ
