Look at coordination of suck:swallow:breathe, and see if baby is happy. 
If normal and happy, there are some rare overgrowth syndromes, but if 
the family history is big babies that thin out, it might just be that 
mom makes lots of milk and babies have good appetites. I've seen 
swallowing difficulties manifest as baby being unable to control the 
flow, keep swallowing, gain rapidly and be fussy all the time.
