okay, I am going to stick my foot into this arena. What matters in the
long run that the mother is happy with the process of restoring the
babies health. Usually with FTT these babies suck horribly. Mothers
during and after birth these days are bombarded with so much. They are
physically and mentally exhausted. Adding a shield over a tube can
unglue a family that is already feeling badly about the FTT diagnosis
and on the borderline about breastfeeding.. Even figer feeding if it is
going to be more than a day or so. I have had families with later
children needing supplemetation state up front NO finger feeding.
Certainly every family deserves to hear all the variety of ways a baby
can be supplemented and they can choose what works best for them(spoon,
cup, dropper, finger, tube, bottle, dental syringe + more). However a
baby with FTT must be supplemented in a timely manner. They do not have
the extra calories to burn. A bottle supplement for a few days is not
the enemy, lack of breastmilk/calories is the enemy So I use what is
called the rules of "F's"
First FEED the baby
Keep Feedings Friendly (ie the "Work" of feeding should be done in about
45 min)breast time+supplement + maternal milk expression- then they can
snuggle, rest and enjoy each other until the next feeding.
Keep the breastmilk Flowing - until there is adequate transfer the
mother must pump and express to rebuild her milk supply.
Encourage the Family that everthing is Fixable as long as the Family
Follow-up with IBCLC care. ( with pre/post weights to wean supplement
and correct any latch issues)
Best Regards
