Esther,

In the management of pain in the post-op lactating breast, you have to take
into account the surgical/incision pain, which can be managed with
anti-pain medication (exactly like a non-lactating operated breast) and the
pain (and possible extra complications) of engorgement if the breast is not
properly drained.

Draining of the post-op breast would either be by baby, hand expression or
pump. Which of these to choose depends on mother´s preference and on where
the incision is. When it is at the areola, we don´t want baby suckling on
it, we don´t want pump flange pressing on it - but there is always a way
around it if you use hand expression.

Basically, as usual in lactation, it is to work with each particular mom
and each particular case.
