Ellen,
It could be a lactose overload.  In addition to the dairy elimination,
you might ask mom to try feeding from one breast at a feeding (or twice
in a row on the same breast if she's already doing one breast) to
increase the baby's fat intake for about 24 hours.  If baby is happier,
and the blood is gone, that could be the answer.

And if it's not a first baby, she could make sure the older sibling has
not "fed" the baby anything...  My 3 year old once fed his 3 month old
sister a peanut when my back was turned for a microsecond.  I heard her
cough and realized what had happened, but it was too late to retrieve it!
