  Please forgive the misspelling of medications.  I am working with a second
time mother- successfully breastfed her first child. With first child problem
was engorgement in both sides-- sore nipples, but no bleeding. Second week of
life got breast infection took dycloxacilin 500 mg 4 times a day-- end of
problems. Second baby now two months of age. History --pain in left breast,
in first week noticed breast tender and red in lower quadrant, no fever--
thought another breast infection and doctor gave dycolxacilin 250 mg 4times a
day. Pain in left breast persists off and on but pain is felt as whole ache
of breast and nipple with some radiating into the arm pit and upper arm. Mom
called another L.C. who said posible  residue infection or ductal yeast.
          ( nipple looks a little red no other signs of yeast)  Mom called
doctor--- took another antibiotic only for 4 days as she had an allergic
reaction -- hives so she stopped med. Breast was having problems empting. Mom
came to me baby has difficulty nursing on left side--- right side seems to do
ok reg cradle hold works well (  possible torticolis?)  Mom feeding baby in
sitting  semi football hold  with baby facing her sitting.  Baby's tongue is
held back so trouble with latch suggest nursing baby in quiet place no tv.
etc.  and curve baby forward more so tongue will drop. Mom took one 200 mg og
dyflucan for ductal yeast --breast seemed to get better for two days and then
severe pain again.  At times baby empties left breast well and othe times
not. At times just finishes nursing and left breast feels OK and then within
5 min knots start forming in upper left breast.  After a very good feeding
mom had worse pain in left breast two hours later ( it woke her up from a
sound sleep-- said it was pain not feeling full and needing to nurse,  Mom is
pharmacist and wants to see study for taking more yeast medication. ( if
anybody has source please send.. OB told her that there is no such thing.) I
suspect that baby is not nursing well and clamping at breast as nipple is
pinched at times , but at the same time when monm feels that she has had a
good feeding she has the worse pain several hours later.  I can work with
baby but my concern that perhaps there is something going on in mom's breast
and the way it is responding to nursing. Mom wants to pump left side to see
if pain continues or spots and then she will know if it is baby or her.
