I have used this method twice this month:

1. Mom had breastfed 2 previous children, but by the time I got to her, 
baby was being bottle fed breast milk (had a good pump from WIC) because
she could never get him to latch on.  I tried to get baby to the breast,
but the arching and pulling away were very vigorous.  Sooo - I took the
bottle she had been using, and sat baby up, demonstrated with the bottle
and bottle nipple what baby needed to do - he had a fast gag reflex and
never took bottle nipple deeply.  I was gradually able to get baby to take
the bottle's nipple deeply, and she observed the long, deep sucks that baby
needs in order to nurse effectively.  You could see it "click" in her
mind...what she had been missing when observing this baby at the breast. 
We discussed how to get baby gently to the breast and how to get him to
take it correctly.  Within 24 hours he was breastfeeding beautifully.

2. Mom was unable to get baby to the breast in hospital, baby was given
many bottles and pacifier.  When I saw the baby (10 days old), mom was
bottle feeding and trying to keep milk supply going with a hand pump.  I
was able to get baby to latch once, but as milk was not flowing, he would
not stay for long, and mom was unwilling to try the SNS.  So I taught her
how to use the bottle so that milk did not flow so fast, and so that nipple
was deeper in the mouth.  After the weekend of this (and mom pumping with a
better pump - and more often than the q 3 hours told be staff in hospital),
she was able, with my help, to get baby to nurse at the breast.  Still
supplemented with bottle for a few more days, but breastfeeding more every
day.  I'm still on this case...
