Libby informed us of cardiologists in her hospital who had claimed to have
seen adverse outcomes in babies fed an improperly prepared formula. It is
very important that health care professionals in the US report such incidents
to the FDA. The FDA administers the Special Nutritionals Adverse Event
Monitoring System. Consumers whose babies have been affected or harmed by
this formula should report this to the FDA at 800 FDA-1088 or at their web
site www.fda.gov/medwatch/report/consumer/consumer.htm

Health care providers should report all formula-related problems to the FDA
MedWatch hotline at 800 FDA-1088 or at the web site
www.fda.gov/medwatch/report/hcp.htm

This helps leave a record of the problem and documents that problems can and
do occur with formula.
